ชื่อของจังหวัดมาจากคำว่า พิษณุ หมายถึง "พระวิษณุ" เทพตามความเชื่อของชาวฮินดู รวมกับคำว่า โลก ทำให้มีความหมายเป็น "โลกแห่งพระวิษณุ" ในสมัยที่ยังปกครองด้วยระบบมณฑลเทศาภิบาล ชื่อของจังหวัดนั้นสะกดว่า พิศณุโลก อาณาเขตติดต่อกับแขวงไชยบุรี ประเทศลาวทางทิศตะวันออกเฉียงเหนือของจังหวัด
